The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking several full-time Support Administrators to facilitate the development of person-centered plans for individuals that promote self-determination. Develop, authorize and revise a personal budget based upon needs and outcomes identified in the plan. Assist individuals and their families to explore and obtain services and supports. Implement an ongoing system of review. The nature of work will include, but is not limited to, the following:
- Act as the primary point of coordination for the person-centered planning team. Utilize an assess/plan/review/revise process to help teams plan for each individual assigned.
- Assist individuals and teams to identify issues, areas of potential risk, and outcomes that are important to and for the individual. Complete and/or coordinate thorough assessments to determine the need for services and supports. Revise assessments as needs change and annually.
- Facilitate the development of a person-centered plan, including the determination and direction of needed supports. Revise plan as needed based on individual or team concerns, progress toward meeting outcomes, plan review, MUI prevention plans, or other issues that impact the need for services as described in the plan.
- Identify funding sources for supports identified in the Individualized Service Plan (ISP) by utilizing a hierarchy of funding options applicable to each person. Plan with teams to access services that are cost efficient and effective to meet the needs described in the ISP.
- Establish and approve budgets for services based upon the individual’s assessed needs and preferred ways of meeting those needs. Manage the fiscal processes related to service authorizations, including use of local and statewide projection tools. Manage processes related to prior authorization as necessary and respond to requests for revisions of authorizations as needed.
- Establish and maintain contact with service providers and natural supports as necessary to ensure that services are effectively coordinated by appropriate providers.
- Promote community presence and participation. Provide referral and linkage to community resources to help individuals meet their needs and to engage with their communities.
- Implement an ongoing system of monitoring continuous plan review to ensure that supports are delivered according to each individual plan, that progress is being made toward desired outcomes and that individuals are satisfied with services. Take action to remediate any immediate concerns regarding health and welfare.
- Assist individuals in choosing providers as needed.
- Assist with enrollment onto Medicaid waivers as appropriate. Complete all related paperwork accurately and in a timely manner to ensure maintenance of funding for supports, including development and recommendation of plans for approval to DODD when services included in plans are funded through the DODD Medicaid Waivers.
- Establish an individual’s eligibility for services of County Board of DD.
START DATE: The Cuyahoga DD hires Support Administrators in groups, or what we refer to as “waves." The next target date for onboarding Support Administrators will be July 15, 2024. The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking an entry level Internal Communications Coordinator with 1 - 3 years of experience to implement agency-wide and departmental internal communications strategies that engage and inspire employees. Such efforts will align with Cuyahoga DD’s vision and will drive employees’ understanding of and their role in helping the agency achieve its strategic goals. The nature of work will include, but is not limited to the following:
- Under a model of collaboration and in partnership with the Director of Communications,engage Cuyahoga DD’s internal departments in the development and implementation of an internal communication plan.
- Manage and maintain the main Cuyahoga DD Intranet page on Sharepoint and support Cuyahoga DD departments in updating and maintaining their individual department Sharepoint pages
- Collect information for, design, and disseminate bi-weekly Catch Up with Cuyahoga DD internal newsletter and assist other departments (i.e., Service and Support Administration and Assistive Technology/Children’s Services) in creating their internal newsletters.
- Support the HR department in their communications regarding employee engagement and events.
- Ensure internal communications messages are consistent across departments and with external key messaging.

ESSENTIAL KNOWLEDGE, SKILLS, & ABILITIES:

The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking a Waiver Coordinator to perform technical and business functions associated with Home and Community-Based Services (HCBS) waivers in order to ensure compliance with applicable rules. Provide technical support to Support Administrators and SA Supervisors in all areas of the DoDD reimbursement system and the DoDD applications portal. The nature of work will include, but is not limited to the following:
- Manage the accurate completion of HCBS authorizations in the DoDD MSS Cost Projection Tool (CPT) submitted by Cuyahoga DD Support Administrators. Provide training and technical assistance to Support Administrator and SA Supervisors regarding completion of the Cost Projection Tool and other documents necessary to maintain HCBS enrollment.
- Preview SA authorizations to ensure the appropriate submission of documents associated with HCBS waivers and local Non-Medicaid funding. Notify and work with Support Administration to resolve problems related to the preview and submission of required HCBS documents and cost projections.
- Manage the authorization of expenditures for HCBS Waivers and local Non-Medicaid funds to ensure fiscal responsibility in DoDD’s Cost Projection Tool, Monthly Rate Calculator and the payment authorization system.
- Assist in the remediation of provider and billing issues Communicate with HCBS providers and DoDD staff in order to resolve PAWS and other associated issues.
- Establish effective relationships and communication with internal customers, DoDD representatives and Primary Solutions staff regarding the Medicaid Services System (MSS), Cost Projection and PAWS-related issues.
- Collaborate with Support Administration and other CMS department staff regarding practice; development, implementation and training of standardized approaches to waiver and non-waiver cost projection tools and provide technical support to the Support Administration staff.
- Prepare and analyze service utilization reports as requested by Community and Medicaid department staff.
- Participate in the Medicaid Administrative Claiming (MAC) program in accordance with DODD rules and regulations and guidance from Cuyahoga DD MAC Coordinator.
- Assist in maintaining accurate data in Cuyahoga DD software packages and with primary data sources, such as DoDD’s Medicaid Service System.

The Executive Division of the Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking an Administrative Assistant who will perform a wide range of secretarial duties, some of which may be administrative or technical in nature. The nature of work will include, but is not limited to the following:
- Type reports, memos, forms, donation acknowledgement letters, and other materials that may be confidential in nature.
- Work with various computer programs and databases.
- Answer telephone and respond to emails; take and relay messages while maintaining confidentiality. Handle routine questions from Board members, staff, parents/guardians, and general public.
- Assist with matters pertaining to Board business and Board meeting preparations using BoardDocs software at the direction of the Executive Administrator Supervisor (EAS), including preparing donation summary memo and meeting agenda items.
- Receive, sort, and distribute incoming mail to the appropriate individuals.
- Prepare materials, some of which may be extensive, for mailings. This includes photocopy and/or fax documents and distribute to individuals on and off-site.
- Oversee agency bulk mailing and business reply mail accounts.
- Maintain files, correspondence, reports, and other documents. Purge and archive files as needed; maintain accurate and detailed record of archived files for retrieval.
- Assist with preparation for presentations and events as needed across various departments. May include securing room, arranging for food/beverages, sending out appropriate notices, taking meeting notes.
- Order supplies for the Executive and Public Information & Communication Departments (PIC) Departments.
- Serve as an active and participating member of the Donzella Building Emergency Response Team.
- Provide backup coverage for the front desk switchboard, and mailroom at the direction of the EAS.

The Cuyahoga County Board of Developmental Disabilities (Cuyahoga DD) is seeking to hire two professionals in one of the following position titles: Licensed Independent Social Worker or Professional Clinical Counselor. Our LISW and PCC professionals provide behavioral assessments and design support strategies for individuals with developmental disabilities, as well as perform comprehensive behavioral assessment and contribute to inclusion of behavioral health strategies in the individualized support plan (ISP). The nature of work for these positions, will include, but is not limited to the following:
- Evaluate for diagnostic, prescriptive, or placement purposes and interpret evaluations for parents, staff or other appropriate parties.
- Make recommendations for program placement, objectives and intervention strategies. (This may include referral to outside community agency.)
- Provide on going consultation as needed.
- Evaluate and conceptualize the needs of individuals on the basis of the individual's history, data, interviews, and direct observation.
- Apply knowledge from the professional literature whenever possible.
- Provide counseling, therapy, and other direct interventions to individuals and families, including services to groups of individuals where appropriate.
- Develop or assist in developing data collection procedures and behavioral programs.
- Monitor implementation of behavior support policies and procedures.
- Trains staff in the implementation of techniques when needed.
- Assists in the collection of program evaluation data in accordance with department or agency direction.
- Provides systematic training for Cuyahoga DD and other staff and parents.
- Maintain all required statistics and records and service-related documentation including but not limited to treatment plans, assessments, and progress notes.
