Overview

The Cuyahoga County Board of Developmental Disabilities is seeking three (3) full-time Support Administrators to facilitate the development of person-centered plan for individuals that promote self-determination. Develop and authorize a personal budget based upon outcomes identified in the plan. Assist individuals and their families to explore and obtain services and supports.  Implement an ongoing system of review.  The nature of work will include, but is not limited to the following:

 

  • Assist individuals and families in identifying issues, concerns, and outcomes that are important to them.  Coordinate further assessment of need for services as necessary. 
  • Facilitate the development of a self-directed person-centered plan.  Revise plan as needed based on individual concerns, monitoring, quality assurance reviews and MUI prevention plans.
  • Assist in determining and directing both the supports and funding needed to reach specific outcomes identified by individuals/families.  Establish budgets for services based upon the individual’s assessed needs and preferred ways of meeting those needs.  
  • Ensure that services are effectively coordinated by appropriate providers. 
  • Assure the health and safety of individuals through shared work with families, technical supports and the community. 
  • Promote community presence and participation.  Assist and collaborate with communities to develop the capacity to support individuals with mental retardation and developmental disabilities. 
  • Implement an ongoing system of monitoring to ensure that supports are delivered according to each individual plan and that desired outcomes are achieved. 
  • Assist individuals in choosing providers as needed. 
  • Identify and/or link the individual to a designated person who is responsible for providing daily representation, advocacy, advice, and assistance, related to day-to-day coordination of services in accordance to the individual’s service plan.  This can be the individual. 
  • Develop and recommend plans for approval to DODD when services and supports included in plans are funded through the DODD Medicaid Waivers. Complete all related paperwork accurately and in a timely manner to ensure maintenance of funding for supports. 
  • Establish an individual’s eligibility for services of County Board of DD. 
  • Attend in-service training, staff meetings, committee meetings, and other meetings as required. 
  • Monitor Services for safety and satisfaction.

 

 

 

Qualifications

  • Bachelor's degree in Special Education, Social Work, Psychology or related area.
  • Either three years of CCBDD experience or four years of other DD experience.
  • Eligible for SSA certification with certification occurring within state statute time lines.
  • Valid Ohio driver's license and a driving record that is insurable under the agency policy; continued maintenance of an excellent driving record.
